Do I need to keep entering my Unique Clinic Logon ID (username) and Password on the Prospective Reporting screen every time I wish to send a Prospective Report?
 

 
No. You only need to enter this logon information once. Once you click Submit Clinic Logon and Password Change after you have entered the correct information, you should not need to re-enter this information unless you reinstall.
