I cannot see the fields to enter my Unique Clinic Logon and Password. What do I do?

You only need to enter this information once. If you cannot see these fields, please click Enter Clinic Logon and
Password, and hit your Tab key until the cursor enters the user name and password fields. Once you hit Submit Clinic Logon and
Password change, your Unique logon information is stored and should no longer be required.
