Your Unique Clinic Logon ID and Password was faxed to you using the number we have on file. This Logon ID and Password is only used for downloading software from this website, and for prospective reporting. If you are unable to logon to SART CORS, please use the Logon ID of sartadmin1 and leave the password field blank. You must then change your password to comply with HIPPA.
